California Healthcare Foundation: Quality Description: A practical tool for addressing errors in hospitals. Critical Incident Reporting System Description: CIRS© is an anonymous and international forum on critical incidents in anesthesiology. This is a reporting system of the Swiss Anesthesia Server, run by the Department of Anesthesia, University of Basel, Switzerland. Its mission is to create an international forum where critical incidents in daily anesthetic practice are collected and distributed. This program not only allows the submission of critical incidents but also serves as a teaching instrument. ECRI Description: ECRI, an international, non-profit, health services research agency and a Collaborating Center of the World Health Organization, is a leading, independent organization committed to improving the safety, efficacy, and cost-effectiveness of healthcare technology, broadly defined to encompass devices, drugs, procedures, facilities, and related standards and guidelines. HCPro Health & Safety Information Description: HCPro is a multidisciplinary, service-based organization focusing exclusively on the healthcare industry. HCPro was created to meet the specialized needs and ever-changing demands that regulations and compliance place on health care professionals. Institute for Safe Medication Practices (ISMP) Description: Medication safety alerts, error reporting, self assessment, news and events. Medical Device Safety Reports Description: ECRI has launched an electronic database to help health care professionals identify hazards associated with various medical devices. The MDSR website provides a selected list of ECRI's hazard reports, Use Experience NetworkTM (UENTM) reports, guidance articles, and frequently asked questions and answers. National Patient Safety Foundation Description: The NPSF is a partnership of health care practitioners, institutional providers, health product providers, health product manufacturers, researchers, legal advisors, patient/consumer advocates, regulators, and policy makers. NPSF is dedicated to the measurable improvement of patient safety through leadership, research support, and education. National Safety Council Description: Mission is "to educate and influence society to adopt safety, health and environmental policies, practices and procedures that prevent and mitigate human suffering and economic losses arising from preventable causes." RMF Center for Patient Safety (Harvard) Description: An internationally renowned leader in evidence-based risk management, RMF acts as a laboratory for reducing medical error and is on the leading edge of patient safety initiatives with its Healthcare Safety and Research Institute.
