Critical Incident Reporting System

Description: CIRSĀ© is an anonymous and international forum on critical incidents in anesthesiology. This is a reporting system of the Swiss Anesthesia Server, run by the Department of Anesthesia, University of Basel, Switzerland. Its mission is to create an international forum where critical incidents in daily anesthetic practice are collected and distributed. This program not only allows the submission of critical incidents but also serves as a teaching instrument.

ECRI

Description: ECRI, an international, non-profit, health services research agency and a Collaborating Center of the World Health Organization, is a leading, independent organization committed to improving the safety, efficacy, and cost-effectiveness of healthcare technology, broadly defined to encompass devices, drugs, procedures, facilities, and related standards and guidelines.

National Patient Safety Foundation

Description: The NPSF is a partnership of health care practitioners, institutional providers, health product providers, health product manufacturers, researchers, legal advisors, patient/consumer advocates, regulators, and policy makers. National Safety Council
